Scheduling an appointment
We ask that you call us to schedule your first appointment at 219-922-8051 during regular business hours. Do not worry if you have to leave a message; our coordinators will call you back promptly. We apologize for the inconvenience, but we are assisting other patients with the same respect and attention we will give you. When you leave a voicemail, please make sure you leave your name and a working number. You can chose which of our three locations works best for you. If you are interested in scheduling an appointment at our Whiting location, please call 219-703-2558. You will need to have your insurance card(s) available for scheduling.

Please arrive 15 minutes prior to your scheduled appointment time and bring the following
When you arrive
When you arrive, sign-in at the front desk. The receptionist will ask you for the above information, and depending on your specific insurance plan, a co-pay or coinsurance payment. This amount is due at the time of service, and it is a federal mandate and office policy that you must pay the amount determined by your insurance plan. You can find out about your insurance plan by visiting the insurance company's website or calling the customer service number on the back of your insurance card. We also have an insurance information tab for further explanation.
You are ultimately responsible for knowing your specific plan and your patient cost. Some insurances, like Medicaid, mandate that you make Dr. Tilak your Primary Care Physician. This must be done before your appointment date to ensure that the insurance covers your healthcare cost.

- current medication list
- any recent lab work
- any pertinent medical or surgical records
